Prepare a Corporate Waiver and Start Slip for Loan-Out on a film or television production in Canada with these template forms. The forms are for contractors who are hired on the production as loan-outs (i.e., loaned out to the film production by the contractor's own company as an independent contractor, and not as an employee of the production company).
